Membership Terms & Conditions
(Monthly & Annual Subscriptions)
1. Membership Overview
Active Life Mobility Massage memberships provide clients with recurring sports massage and recovery services at a discounted rate. Memberships are available on a monthly or annual basis and automatically renew unless canceled according to the terms below.
2. Billing & Payments
• Monthly memberships are billed automatically every month on the sign up date.
• Annual memberships are billed automatically once per year.
• A valid debit/credit card must remain on file for the duration of the membership.
• All payments are NON-refundable, including unused sessions or early cancellations.
3. Included Sessions & Booking
• Members must schedule appointments in advance through our booking system.
• Sessions included in each membership must be used within the billing period and do not roll over unless otherwise stated in the membership description.
• Members receive priority appointment access, including:
• Booking up to 1 hour before scheduled opening hours
• Appointment overflow up to 30 minutes after scheduled closing hours
(Based on availability)
4. Cancellations & Missed Appointments
• A minimum 24-hour notice is required to cancel or reschedule a session.
• Missed appointments or late cancellations will result in forfeiture of the session. Or you have the option to pay $40 to retain your monthly session for that month.
• Repeated no-shows may result in suspension or termination of membership.
5. Membership Changes & Pauses
• Monthly members may request to pause their membership once every 6 months for up to 30 days.
• Annual members may pause up to two times per year for up to 30 days each.
• Pauses must be requested via text or email and confirmed by the office.
• Changing membership tier (session length or frequency) can be done at any time and will apply to the next billing cycle.
6. Cancellation Policy (Membership)
• Monthly memberships may be canceled anytime with a 30-day written notice (text or email).
• Annual memberships may be canceled with a 60-day notice, but refunds are not issued for unused portions.
• Any outstanding balances or fees must be paid before cancellation is processed.
7. Sharing & Transferability
• Memberships and Annual Plans may only be used by the member on file.
• Family or partner sharing is allowed unless purchasing a designated single membership option (if offered). We will let the customer know which plans may be used for family members.
8. Price Changes
• Active Life Mobility Massage reserves the right to adjust membership pricing with a 30-day notice to all active members.
• Members may cancel before the new billing cycle if they do not wish to continue at the updated rate.
9. Member Conduct
• Respectful behavior toward staff and other clients is required.
• Inappropriate behavior, harassment, or violation of policies may result in immediate termination of membership without refund.
10. Agreement
By purchasing a membership, the member agrees to all terms listed above and authorizes Active Life Mobility Massage to charge the card on file for recurring membership dues. Or for a one time payment for our Annual plans of care.
Priority Appointment Terms:
Our priority appointments are only available for monthly membership, annual membershi, and package holders ONLY!
As a valued Monthly or Annual Member, you receive Priority Scheduling Access with the following exclusive booking privileges:
1. Extended Booking Hours:
Members may schedule appointments up to 1 hour before regular opening hours and up to 30 minutes after regular closing hours, based on therapist availability.
2. Advance Notice Required:
Early or late appointments must be requested at least 12 hours in advance to ensure therapist availability and proper scheduling.
3. Limited Availability:
Priority appointments are subject to staff and facility availability. We will make every effort to accommodate your preferred time, but these bookings are not guaranteed without confirmation.
4. Cancellations & Reschedules:
Early or late appointments follow the same cancellation policy as standard sessions. Missed appointments or same-day cancellations will result in a session being deducted from your membership or package. Or you will have the option to pay a ($40) same day cancellation fee
